MORE THAN A MEAL CAMPAIGN
Parents/Guardians of students are asked to complete one Household Income Form for School Program Funding for each child enrolled who is NEW TO THE DISTRICT.
This form has important benefits and can result in important resources for our school.
The form can be completed confidentially online at Household Income Form Process. Up to six children can be submitted on one online form. You may also call our school at (323) 733-1178 and ask for Ms. Alvarez, Coordinator, to complete a form.
The information collected on this form is 100% confidential and not shared outside the District.
